When you purchase $30 in Olay products, you will get $10 back by mail!
Submit the rebate form and original register receipt with item(s) and purchase price(s) circled and enter the UPC codes in the spaces provided on the form. Request must be submitted by September 15, 2012.
Purchases need to be made between July 1, 2012 and August 31, 2012. Allow 6-8 weeks for delivery of $10 rebate in the form of a pre-paid cash card.

I can not find the Olay rebate form for the $10 0n $30. The rebate form that comes up is the $10 on $20.
Can anyone help me.

Where can i get the forms. everyone that comes up is either expired or is for $10.00 with $20.00 in covergirl or olay products. I cannot find the form for this rebate.
